How to receive support under the NDIS

The National Disability Insurance Scheme (NDIS) provides support to people with a disability. It means greater choice and more control for people to achieve their goals.
St Agnes’ Care & Lifestyle is registered to provide services to NDIS participants under the name of Catholic Parish of St Agnes Port Macquarie.

1 Firstly check if you are eligible to receive support through the NDIS. You must live in Australia and aged between 7 and 65 and have a disability that requires support from a person or equipment. Click here to see the NDIS checklist in detail.

2 If you are eligible, call 1800 800 110 and ask to make an Access Request. You will be sent an Access Request form which you must complete. You can contact us if you need help completing the Access Request form.

3 The Access Request form will ask you questions to confirm your identity, eligibility, evidence of your disability and consent for the NDIS to seek further information.

4 The NDIS will decide if you are eligible to receive NDIS support. If you are eligible, a planning meeting will occur to determine how much support and the type of support you require.
