How to assess your Home Care needs
- Have you noticed a change in what you can do or remember?
- Have you been diagnosed with a medical condition or reduced mobility?
- Has there been a change in family care arrangements?
- Have you had a recent fall or hospital admission?
Family, friends or an advocate can help you establish your Home Care needs and once these are identified you can select the services required to meet those needs.
Your eligibility is assessed based on these needs and a Government assessment.
