St Agnes' Care and Lifestyle - Donnelly House

Getting Started with Residential Care

What help do you need and how can you get it?

We understand that the process of entering residential care and accessing subsidised funding can be confusing so we have six easy steps to get you started.

1 Discuss with your family, friends and doctors your lifestyle options and the specific health care you need.

2 Book an appointment with the local Aged Care Assessment Team (ACAT) by contacting www.myagedcare.gov.au or call 1800 200 422. An ACAT aged care specialist will visit your home and ask you about your specific care needs. Your family, friend or advocate can be there with you.

3 Our Client Relationship Team will help you with how to access your subsidised Government funding, by providing access to the Income and Asset Test Centrelink Form. This may have an impact on your residential care accommodation costs.

4 A letter from My Aged Care will be sent to you with a referral code. You have been approved entry into an aged care home.

5 Make an appointment with our Client Relationship Team to progress your application. Bring your letter from My Aged Care with the referral code.

6 Accept an offer and sign your individual Residential Aged Care Agreement. This outlines your services, accommodations costs with your rights and responsibilities.
