National Disability Insurance Scheme (NDIS)

The National Disability Insurance Scheme (NDIS) is a Federal Government program to support people with a disability. It means greater choice and more control for people to achieve their goals.

St Agnes’ Care & Lifestyle is registered to provide services to NDIS participants under the name of Catholic Parish of St Agnes Port Macquarie. Registration Number: 4050059727.

We offer person-centred, tailored support to NDIS participants in the following ways

Plan management
Plan Managers ensure all NDIS supports a participant uses are paid for in a timely manner, and within budget. They support information to the participant on remaining funds and services paid.

Support Coordination
Support Coordinators assist people to understand their NDIS plan, connect and coordinate their supports, and develop capacity and resilience to manage their plan into the future. A Support Coordinator can assist with the implementation of supports in the plan, coordinate assessments, reports and service proposals, support the participant through identifying and selecting providers, monitoring plan and outcomes and plan expenditure.

We have offices in Port Macquarie and Kempsey providing outreach to Port Macquarie, Kempsey, Wauchope, Laurieton, Taree, Nambucca Heads and Macksville.

NDIS Code of Conduct

St Agnes’ Care & Lifestyle aims to uphold the rights of people with disability, including the right to dignity and respect, and to live free from abuse, exploitation and violence. The NDIS Code of Conduct is a set of rules to help make this happen.

All workers must follow the NDIS Code of Conduct.

This means that your worker should be someone who:

  • Treats you with RESPECT and gives you FREEDOM to make your own decisions.
  • Respects your RIGHT to PRIVACY.
  • Is COMPETENT and SKILLED.
  • Has INTEGRITY and is HONEST.
  • Ensures you are SAFE and acts quickly if there are risks or incidents.
  • Helps to PREVENT violence, abuse or neglect.
  • Acts PROFESSIONALLY and ETHICALLY.

It is very important to us that you are happy with our services. If for any reason you are unhappy about your current service, it is important you talk about it.
