Getting Started with Home Care

We understand navigating a Home Care assessment can feel overwhelming. That is why we have broken it down into seven steps on how to begin your home care journey.

1 Contact our Client relationship Team to discuss the Home Care services or help you need at home.

2 Our Client Relationship Team will help you register with My Aged Care online.

3 Speak to our Client Relationship Team to help you book an Aged Care Assessment Team (ACAT) appointment where an assessor will visit your home and ask you about the help you need.

4 Our Client Relationship Team will assist by providing the Income and Assets form required, which will determine your eligibility for Government funding, and what, if anything your contributions will be (Income-tested fee).

5 A letter from My Aged Care will be sent to you with a referral code.

6 Our staff will provide you with information regarding Home Care Services. Meet with the Client Relationship Team and discuss your personal needs and schedule preferences.

7 Sign your individual Home Care Services Agreement confirming when package commences and meet your dedicated Home Care Coordinator.

How to assess your Home Care needs

  • Have you noticed a change in what you can do or remember?
  • Have you been diagnosed with a medical condition or reduced mobility?
  • Has there been a change in family care arrangements?
  • Have you had a recent fall or hospital admission?

Family, friends or an advocate can help you establish your Home Care needs and once these are identified you can select the services required to meet those needs.

Your eligibility is assessed based on these needs and a Government assessment.
